Birth and Death Records

New!  Birth and Death Certificates can now be paid online through our website.  You must FAX the Application for Certified Copy of Birth or Deat Certificate to 360-676-6771 in order to obtain your certificate.  Once payment is received, FAXED applications will be processed the following business day.Office Location and Hours

Applications received in person by 11:30 AM will be available by 2:00 PM, same business day.
Applications received in person after 11:30 AM will be available by 9:00 AM the following business day.

General Information about Birth Certificates

Certified birth certificates can be obtained from our Vital Records department for:

  • All Washington State births registered from July 1, 1907 to present.

What information is Needed to Obtain a Birth Certificate?

  1. Full Name of Child (First, Middle, Last)
  2. Birth date of child
  3. Place of birth
  4. Mother's maiden name
  5. Father's full name (if listed)

How Much Do Birth and Death Certificates Cost?

  • $20.00 each effective July 22, 2007. Payable in US Dollars ONLY
  • Note: If we are unable to find your record, you will be charged an $8.00 fee for the search. The charge is per RCW 70.58.107.
  • We accept cash, debit or credit cards. There is a convenience fee of a minimum of $2.00 to a maximum of 2.35% on debit and credit card orders.

How Do You Obtain a Birth Certificate?

  1. By FAX
    1. Download the form: Application for Certified Copy of Birth Certificate

    3. Fax the completed application to (360) 676-6771
      **if payment is received by 11:59pm, the order will be processed the following business day.

  2. By Mail
    1. Download the form: Application for Certified Copy of Birth Certificate
    2. Mail the completed application along with a money order made out to Vital Records to the address at the top of this page

  3. In Person
    1. Download the form: Applicaiton for Certified Copy of Birth Certificate
    2. Bring competed applicaiton along with cash, money order, debit or credit card to the address at the top of the page.
